Thopaz+ (Medela UK) is a portable digital chest drain system that provides regulated negative pressure close to the patient’s chest and continuously monitors and records air leak and fluid drainage.
What is Thopaz?
Thopaz chest drain system is a portable suction unit which allows mobilization of the patient (Figures 1). This offers patients the benefit of suction and enables early mobility. It has scientific digital flow recordings with an in built alarm system (Figure 2).
What does an air leak indicate?
An air leak is the leakage of air from an air-containing cavity into spaces that, under normal circumstances, do not contain air. The term air leak syndrome is used to describe symptoms of respiratory distress occurring in conjunction with an air leak.
Can you have an air leak on water Seal?
Air bubbling in the water seal chamber indicates an air leak. The third chamber regulates the amount of suction applied through the system. Typically, there is an air leak meter that indicates the degree of air leak, measured in columns from 1 to 7.

How does a chest drain system work?
Chest drains also known as under water sealed drains (UWSD) are inserted to allow draining of the pleural spaces of air, blood or fluid, allowing expansion of the lungs and restoration of negative pressure in the thoracic cavity. The underwater seal also prevents backflow of air or fluid into the pleural cavity.
What is a one way Heimlich valve?
The Heimlich valve is a small one-way valve used for chest drainage that empties into a flexible collection device and prevents return of gases or fluids into the pleural space. The Heimlich valve is less than 13 cm (5 inches) long and facilitates patient ambulation. Currently there are several systems in the market.

The most common cause of air leak syndrome in neonates is inadequate mechanical ventilation of the fragile and immature lungs.
What is the difference between suction and water seal?
If a chest tube is placed, gravity water seal drainage is used, and suction is only added if the lung does not re-expand as quickly as expected. The tube allows air and/or fluid collection to drain out of the body resulting in re-expansion of the patient’s lung.
What causes air leak in lung?
Pulmonary air leaks occur when there is uneven alveolar ventilation, air trapping and high transpulmonary pressure swings. In the immature lung, the pores of Kohn are reduced and thus uneven ventilation is compounded by a lack of redistribution of pressure through the alveolar connecting channels.
